Details:   The Service Provider will be required to provide Construction Contract Administration Services for Contract 2012-5109 on Highway 11, from 7.6 km South of Jct. Hwy 11B Northerly for 8.0 km. The highway reconstruction contract includes Grading, Drainage, Granular Base, Hot Mix Paving, and Structures.
Other Information:   There will be no EOI posting for this Request for Quotation Submission.

Interested firms who meet the qualifications listed in the Project Notice including the Prime Consultant approved in RAQS Prime Speciality listed can submit a quotation for the above noted assignment. Also, a firm must have the prior registration of their Core Plan and Generic Category Plan for the Category where the Prime Specialty is located. Firms that do not meet the qualifications specified will be disqualified from further consideration.

A Transmittal Letter must be included indicating the firmâ¿¿s intention to bid on the project and indicating the name, title, address and telephone number of the Principal who will serve as the contact for the project. The letter must be signed by the Key Contact or an Alternate Contact who is listed in RAQS, and who is empowered to sign a Legal Agreement with the ministry and make decisions for the firm on contractual matters.

The RFQ submission for this assignment must also include a signed, completed RAQS Declaration Form. The form is posted on the Consultant Information Web page.

The RFQ will be comprised of three separate Parts:
Part A - RFQ Terms of Reference
Part B - RFQ Terms and Conditions and
Part C - RFQ Forms and Notices
The three Parts combined will form the RFQ Agreement (Interested Proponents are to download and print the Parts A, B and C)

Part A - contains project specific requirements and is posted to this Project Notice. Interested Proponents are to download and print Part A and review the Terms of Reference including submission instructions.

Part B - contains the standard agreement terms and conditions. Part B is posted on the RAQS Consultant Information Web Page. Interested proponents are to download Part B and review the terms and conditions.

Part C - contains Notices and required submission forms that will form the RFQ Agreement. Part C is posted on the RAQS Consultant Information Web Page. Proponents are to download Forms and Notices from this page.

Interested Proponents are responsible to review Parts A, B and C of the RFQ and familiarize themselves with the Terms of Reference, the Terms and Conditions and the Forms and Notice pertinent to the assignment.

Failure to submit by the date and time indicated, or failure to submit the required copies in the required format will result in disqualification.

Failure to submit a Form 6 as required (see Part A) will result in the submission not being considered by MTO

All submissions must be clearly identified as "confidential" on the envelopes. All submission envelopes must show the Assignment Number and Project Name and must be addressed to, as specified in Part A of the RFQ.
Disclosure of information relative to the acquisition process will be provided in writing, at the award stage, to all submitters. For RFQ assignments, the name of all bidders, the winning bidder and winning bidders quotation will be released.
